This form must be filled out by any non-member coming to the Y. Parent / guardian signature is required for a minor. Please print this out and send it with if parent / guardian will not be with the child.
This form is to be completed by the parent / guardian of any child enrolled in a youth sport or aquatics program. It also gives facts and symptoms of concussions. Please remember that concussions can happen in any sport.
